Complete Medical Record & History Review: A detailed analysis of your medical history, diagnoses, and treatments to identify gaps and ensure accuracy.
Preventative Health & Safety Audit: An in-depth evaluation of home safety risks, lifestyle needs, and preventative health measures to prevent avoidable ER visits.
Emotional & Cognitive Health Assessment: A sensitive, targeted evaluation of cognitive well-being and emotional health to ensure holistic support.
The Master Navigation Plan: A personalized, easy-to-follow care plan summary that provides an actionable roadmap for you, your family, and your providers.
Note: Completion of the Care Navigation Assessment is required prior to starting any Ongoing Health Coordination or Concierge services.

Continued Care Management: Healthcare doesn’t stop after an initial assessment-and neither does our support. Ongoing Health Coordination gives you dedicated access to professional care management on a schedule that fits your life.
Our packages start at $100/month for our Standard Support Tier, which includes monthly check-in calls — perfect for predictable, steady support.
The Enhanced Support Tier ($200/month) provides more frequent weekly or biweekly guidance during periods of higher clinical needs or for managing complex care needs that require closer oversight.
Need extra support on-demand? You can also schedule a la carte consultations as needed available in flexible 30-minute ($50) or 60-minute ($100) sessions.
Expert "eyes and ears" on the ground, especially for adult children living out of town, including in-person medical visits, priority phone consultations, and robust case management.

Nurse Advocate Concierge
What’s included:
Medical Appointment Partner: We attend the specialist appointments with you or your family member and serve as your dedicated healthcare advocate, asking the tough questions, keeping precise clinical notes, and streamlining coordination among your healthcare team. Monthly fee includes one specialist appointment per month. Add-on coverage of $50 per additional healthcare provider appointment in the same month.
Multidisciplinary Network Coordination: Get the most out of your care plan through coordinated expertise. Enjoy direct access to trusted clinical partners, physical therapy, and pharmacy medication reconciliation tailored to bridge the gaps in your care and ensure your treatment plan is as effective and personalized as possible.
Long-Distance Family Connection: We provide local clinical oversight for out-of-town families and connect with you monthly in a 60-minute care plan overview. Let us be your eyes and ears on the ground-ensuring your loved one receives high-quality care while giving you total visibility.
